# Enabling a Value-Based Care System Through Technology

A large academic medical center wanted to enable the capability of a value-based care system through technology. Additionally, the adoption and utilization of new processes and tools were required to understand preventative interventions and the relationship between cost and utilization.

The Approach

# From Building the Technical Foundation to Ensuring Stakeholder Adoption

Our team built the technical foundation and aligned data integration and requirements. We structured the framework to consume external data, produce ad-hoc physician panel-level reporting, and identify 16 critical quality measures for intervention. Once user profiles were established and the impact was assessed, our team collaborated with key stakeholder groups to understand use cases and motivate adoption.

# Delivering Proactive Processes in a Shifting Landscape

Within a shifting landscape of alternative payment model changes and participation challenges, we integrated and utilized over 2M claim files in the measure calculation, established new processes to allow proactive patient contact, engaged 215 physicians in the new process, and created dashboards to understand pacing at multiple levels.
